What are the responsibilities and job description for the Hospice Social Worker position at Soulistic Hospice?
Soulistic Hospice is a growing nonprofit organization in need of a social worker to provide services to hospice patients and their families, including initial and ongoing emotional, spiritual, psychosocial assessments, counseling and bereavement services.
Soulistic Hospice provides a healthy positive work culture and has been under the same management for 18 years.
Bilingual Spanish/English preferred.
Social Worker Responsibilities:
- Make visits, phone calls, and document to our standards. We will teach you the Soulistic Way using the most thorough training systems in Hospice. We will never put you in a position where you cannot succeed.
- Make visits when it’s best for patients. We have a standard number of visits we want you to make within a range—not too few, not too many. We won’t micro-manage your hours or days.
- Attend our regular virtual Interdisciplinary Team & Social Service Team meetings

Pay: $30.00 - $35.00 per hour
