About This Role:
The LPN Medical Team Lead is a key member of our nursing team, responsible for overseeing daily operations and ensuring the delivery of high-quality care to our residents.
Duties and Responsibilities:
- Supervise nursing staff in the delivery of resident care
- Monitor and schedule job assignments, developing unit priorities as needed
- Participate in care plan meetings, develop and implement resident plans of care
- Evaluate resident response to interventions and alter care plans through ongoing assessments
- Care for residents using nursing judgment, following policy and procedures
- Monitor residents' overall mental and physical status for changes, reporting condition updates as necessary
- Complete quality care audits and ensure adherence to procedures regarding hazardous chemicals and fire safety
- Provide direct resident care when needed, completing treatments, procedures, and administering medications as ordered by physicians
- Document all pertinent information on interdisciplinary notes, medication administration records, and treatment logs, protecting resident and employee confidentiality
- Discharge, transfer, and admit residents as required
- Ensure compliance with regulations from DHEC, Fire and Safety, OSHA, Labor Laws, and HIPAA standards, respecting resident rights
Requirements:
- LPN or RN certificate
- Current SC LPN or RN license
- At least one year of experience in geriatric nursing, including medication administration, delivery systems, and IV therapy
- Basic life support (BLS) and CPR certification
- Prior supervisory/management experience, with skills in staff development, training, and scheduling preferred
- Understanding of Medicare, Medicaid, DHEC, and other relevant laws and policies, as well as resident rights
- Ability to make sound, independent decisions and maintain patient tactful composure in challenging situations
- Strong communication and motivational skills, including problem resolution in resident care and employee management
Work Environment:
This role requires the ability to lift 50 pounds or more, exposure to biological agents such as viruses and other microbes, and scheduled work during off-hours, including weekends, nights, or holidays.
