Position Summary
The Surgical Navigator is a registered nurse aids patients, families, and caregivers to promote care coordination and remove potential barriers to access to oncology care. Through the surgical continuum of care and ensuring that each patient receives comprehensive education based on their needs and applicable related condition(s). This person manages the on-going analysis, development, and implementation of methods and systems to improve the overall efficiency and effectiveness of the program’s clinical functions; coordinates patient-specific and community outreach; and is accountable for the surgical specialties program results through coordination of care. The coordinator contributes to patient services and the hospital through the support of all corporate objectives.
The oncology nurse navigator aids patients, families, and caregivers to promote care coordination and remove potential barriers to access to oncology care. The Oncology navigator’s primary function is to facilitate informed decision making and timely access to high-quality care throughout the cancer continuum. Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging, dietary, and others. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process.
Qualifications
· Registered Nurse with current unrestricted RN license in the State of South Carolina
· Minimum of five years recent acute hospital experience
· BSN or Master’s in Related field
· Excellent written communication skills and knowledge of electronic record documentation
· Support group leadership experience preferred
· Must have current BLS
