What are the responsibilities and job description for the Adult Care Coordinator Coffee County Full Time position at SOUTH CENTRAL ALABAMA MENTAL HEALTH BOARD?
Employees in this position are expected to obtain relevant information from adults, write plans of care, establish liaisons with other service agencies in the community, and assist consumers in obtaining the services they are entitled to receive. Must have Bachelor’s degree in Human Services field; must have current driver’s license for the last three years with an excellent driving record, eligibility for coverage under center liability insurance and individually held vehicle liability insurance and clear background check.
