Position Overview: We are seeking a dedicated and passionate Developmental/Intellectual Disability Care Coordinator (DD/ID) to join our team in Coffee County. In this role, you will conduct home visits, obtain relevant information from adults with developmental/intellectual disabilities and write plans of care. You will also establish liaisons with other service agencies in the community and assist individuals in obtaining the services they are entitled to receive.
Qualifications:
- Must have a Bachelor's degree in a mental health-related field.
- DD/ID Case Management training preferred.
Responsibilities:
