Description
1. Provide direct support and assistance to consumers in all life areas as determined by their needs and desires.
2. Demonstrate support and promotion of personal outcome measures and commitment to the mission, values and principles of the agency.
3. Maintain knowledge of each consumer’s goals and desires in order to provide each individual with the necessary supports needed to achieve these goals and desires.
4. Actively aid each person to receive all supports needed from both within the agency and community resources. Encourage choice making through experience broadening activities on a consistent basis.
5. Support each consumer to live self-determined lives by providing choice in all areas including use of their money, work, food, medical care, daily schedules and social activities, relationships with others, etc. while at the same time helping each person to understand the balance between rights and responsibilities.
6. Ensure that each consumer’s affiliation with people, organizations and services meet his/her needs and desires. Actively promote and support involvement with the person’s community and external supports (family, friends, other organizations) as desired by the consumer.
7. Ensure proper hygiene and appropriate personal appearance of consumers as they go into the community.
8. Provide transportation in agency vehicles as needed.
9. Monitor and promote the personal health and wellness of each consumer by supporting the individual in receiving all needed health/mental health and wellness interventions. Provide transportation to medical appointments as needed. Support each person to understand the status of his/her own health, what options are available and how and when to take personal medications.
10. Assist consumers to take medications as prescribed and complete documentation accurately; maintain authorization to assist with medications according to agency policy.
11. Maintain knowledge of policies regarding abuse, neglect and mistreatment of consumers. Report allegations of abuse, neglect or mistreatment of consumers according to agency policy; cooperate with investigations related to any such allegation.
12. Maintain environment of homes (both inside and outside) in a safe and clean condition as required by the agency and its governing bodies. Notify appropriate staff in a timely manner to report problems related to safety and home maintenance.
13. Demonstrate an understanding of human rights and due process in order to support each consumer to exercise his/her rights. Be aware of agency policies and procedures regarding rights and support consumers to follow due process if the need arises.
14. Represent the agency to neighbors, the community, families, etc. in a positive and respectful manner. Show respect for consumer’s and the agency’s property.
15. Complete all documentation in a timely and legible manner as required by the agency and its governing bodies.
16. Guarantee the safety and well-being of each consumer by providing supervision based on the individual’s needs as determined by licensure requirements. Provide direct supervision at all times while in and out of the home. Respond to the needs of customers in a timely manner.
17. Meet all agency training requirements.
18. Work in cooperation with others and consistently promote a positive workflow.
Requirements
1. High School Diploma and experience working with individuals with Intellectual /Developmental Disabilities.
2. Aptitude for developing relationships with adults with Intellectual /Developmental Disabilities.
3. Must have good writing skills.
4. Must be able to complete DSP training and medication within 120 days of employment.
5. Maintain a valid driver’s license; has an acceptable driving record as determined by agency auto insurance carrier (insurable
under agency auto policy); has a reliable, insured car available to use during work hours.
