Job Description
We are seeking a highly skilled Senior Patient Care Coordinator to join our team at South Shore Health. As a Senior Patient Care Coordinator, you will act as a patient advocate and social worker to our clients, working in coordination with the RN Case Manager to coordinate services and resources for transitional care planning.
Key Responsibilities
- Review work lists and census to prioritize patients and identify those that meet criteria for social work interventions.
- Initiate referrals from the Case Manager or self-referral when appropriate.
- Coordinate with the Case Manager using high-risk criteria to determine the plan for completing initial discharge planning assessments on all patients.
- Begins discharge planning discussions with patients and families.
Requirements
- A BS in Social Work; Master's prepared LICSW or LCSW preferred.
- Recent healthcare experience or related field preferred.
