Job Summary:
The Service Coordinator is responsible for establishing for and assuring that residents are linked to supportive services. Additionally, the Service Coordinator is responsible for monitoring the provision of these services as an appropriate means of supporting the individual to continue to live independently.
Job Duties/Responsibilities:
1. Provides general case management and referral services to all residents needing assistance
2. Completes intake on new move-ins to the housing program to identify needs and goals for living independently
3. Provides formal case management including; evaluation of health, psychological, and social needs, using an acceptable assessment tool
4. Establishes linkages with all agencies and service providers in the community
5. Maintains an up to date directory of service providers for use by residents, family members and staff
6. Makes referrals to service providers in the community; these referrals may include formal assessment, personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurse, preventive/health screening, wellness programs, legal or other advocacy
7. Develops case plans in accordance with formal assessment services
8. Provides education to residents on service availability, application procedures, client rights, etc
9. Maintains necessary case files on each program participant/resident
10. Monitors the ongoing provision of services from community agencies and keeps the care management and provider agency current with the progress of the participant/resident
11. Serves as staff to the PAC
12. Sets up volunteer support programs with service organizations in the community
13. Helps the residents build informal support networks with other residents, family and friends
14. Performs other duties as assigned
Required Education, Training, and Experience
1. A Bachelor of Social Work (BSW) or degree in Gerontology, Psychology or Counseling is preferable, a college degree is fully acceptable
2. 2-3 years experience in social service delivery with seniors
3. Demonstrated working knowledge of supportive services and other resources for senior citizens in the community in which the program is located
4. Ability to advocate, problem solve, apply critical thinking and good judgement and provide results for the elderly served
5. Strong writing and organizational skills
Required Skills and Abilities
Ability to read, analyze and interpret general business periodicals, policies and procedures and regulations
Ability to write monthly and quarterly reports and develop and revise necessary policies and procedures
Ability to input clinical information into computer
Ability to calculate simple figures, costs and percentages
Excellent interpersonal, communication, and organizational skills
Excellent phone etiquette
Job Type: Full-time
Pay: $24.50 - $28.85 per hour
