Doc-Only Case Manager
Description
Case management position that engages and assists patients who are seeing a psychiatric provider only. Duties include treatment plan updating, completing discharge summaries, referring patients to a case management treatment team when necessary, and offering in-office or phone support when patients are seeking assistance or have questions.
Position Requirements
AAS / BA required.
