Description
Southeast Primary Care Partners seeks a Value-Based Care (VBC) Specialist to lead the implementation of value-based care initiatives across assigned practices. This role requires a collaborative professional with expertise in HEDIS/STAR measures, risk adjustment (HCC), and healthcare analytics. The specialist will drive quality improvement, optimize patient outcomes, and ensure adherence to OSHA/EEOC standards while working closely with providers and staff. Multiple positions (N. GA, Anniston, AL).
Requirements
Key Responsibilities
- Practice Engagement & Relationship Building
- Serve as the primary liaison for assigned practices, building rapport with providers, office staff, and leadership to foster trust and collaboration.
- Facilitate meetings to align stakeholders on value-based workflows, clinical documentation accuracy, and EHR optimization.
- Performance & Quality Improvement
- Analyze HEDIS/STARS data, HCC risk scores, and cost/utilization trends to identify care gaps and develop targeted interventions.
- Deliver provider education on risk adjustment coding, quality metrics, and documentation best practices.
- Operations & Workflow Efficiency
- Conduct practice assessments to address inefficiencies and implement workflow improvements.
- Partner with practices to integrate patient outreach strategies (e.g., preventive screenings, chronic care management).
- Regulatory Compliance
- Ensure compliance with OSHA standards by promoting workplace safety protocols and addressing hazards.
- Uphold EEOC guidelines by fostering an equitable, inclusive environment for staff and patients.
- Cross-Functional Leadership
- Collaborate with the VBC Manager, Medical Director, and Executive Leadership to meet regional project deadlines.
Qualifications
- - Required:
- 2 years of experience in value-based care, HEDIS/STARS, or risk adjustment (HCC).
- Proficiency in healthcare analytics tools and EHR systems (e.g., Epic, Cerner).
- Strong knowledge of CMS quality programs and clinical documentation standards.
- - Preferred:
- Certification as a Medical Assistant, RN/LPN, or coding credential (CPC, CRC).
- Experience working directly with primary care providers and multidisciplinary teams.
- Demonstrated success in improving STAR ratings, HCC capture rates, or cost-of-care metrics.
Travel Requirements
- On-site presence required 2-3 days per week to support assigned practices.
- Regional travel to offices within the territory (mileage reimbursement provided).
