Job Summary:

We are looking for someone who is passionate about improving community health, has excellent communication skills, and is dedicated to helping others. As a Community Health Worker, you will play a vital role in connecting health providers with community resources, educating patients on reducing health risk factors, and empowering individuals to take control of their health.

In this role, you will facilitate communication between patients and healthcare providers, troubleshoot barriers to care, engage with the community through outreach, and assist in developing care management processes. You will also build community capacity by increasing health knowledge and self-sufficiency, collaborate with care teams to overcome healthcare barriers, and connect patients with necessary resources.

Key Responsibilities:

  • Facilitate communication and patient empowerment in interactions with healthcare and social service systems
  • Troubleshoot barriers to care and engage high-risk patients through community outreach
  • Serve as a liaison between health/social services and the community to improve service delivery
  • Assist in developing care management processes to improve self-management of chronic disease
  • Build community capacity through education, social support, and advocacy
  • Collaborate with care teams to create solutions for healthcare barriers
  • Connect patients with community resources and health center services
  • Follow up with patients via phone calls, home visits, and other settings
  • Conduct public education activities and facilitate enrollment in qualified health plans
  • Provide culturally and linguistically appropriate information to the community

Education, Experience, and/or Credential Qualifications:

  • Bachelor’s degree from an accredited college or university in a related field preferred
  • High School Diploma or equivalent required
  • Two (2) years of experience in community health, human services outreach, or healthcare preferred
  • Desirable experience includes customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, and public speaking

Additional Qualifications:

  • Must provide evidence of being free of infectious and contagious diseases prior to employment and annually thereafter
  • TB testing expected for all direct care staff
  • Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check
  • Completion of New Hire Orientation and all training requirements including Relias
  • Current driver’s license, acceptable driving record, and current auto insurance
  • Achievement of required training within specified timeframes
  • Valid Driver’s License in state of residence

Physical Requirements:

  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects
  • Sedentary work involves sitting most of the time but may involve walking or standing for brief periods
