Job Summary:
The Intake Coordinator plays a vital role in facilitating the admission process for patients transitioning to home health care services. This position requires excellent communication skills, ability to multitask, and the ability to work collaboratively with healthcare professionals and patients.
QUALIFICATIONS:
1. Experience with electronic health records (EHR) and healthcare management systems.
2. High School Diploma, GED or higher.
3. Excellent communication and interpersonal skills.
4. Strong organizational and multitasking abilities.
5. Ability to work independently and as part of a team.
6. Proficient in Microsoft Office Suite (Word, Excel, Outlook).
RESPONSIBILITIES:
Receive and process referrals from hospitals, physicians' offices, and other healthcare providers.
Verify insurance coverage as needed and obtain necessary authorizations for home health services.
Review progress notes/encounter visit notes for accuracy and completeness according to
regulatory requirements to facilitate timely processing of plans of care.
Act as a liaison between agency and other healthcare providers to request/obtain additional documentation for admission.
Ensure all referral documentation is complete, accurate, and compliant with regulatory requirements.
Communicate effectively with patients and their families to gather necessary information for admission.
Maintain communication with referral sources to facilitate smooth transitions and ensure continuity of care.
Document all intake activities accurately and timely in electronic health records (EHR) systems.
Reviews and corrects Medicare eligibility checks weekly including overlapping episodes, and transmission errors.
All other duties assigned.
REQUIREMENTS:
Knowledge of HIPAA regulations and patient confidentiality practices.

Ability to prioritize tasks and work efficiently in a fast-paced environment.
Home Health experience.
Wellsky, Navihealth, Careport, Ensocare experience a plus.
Billingual a plus.
