Southern Coos Hospital & Health Center has an opening for an onsite Case Manager/Discharge Planner/UR Coordinator. The primary responsibilities of this position include obtaining prior authorizations for all Inpatient and Swing bed patient stays, maintaining swing bed patient records in compliance with regulations for documentation of care and services; and working to ensure admission, continued stay, and discharge requirements are met for all patients served on the Inpatient unit; coordinating on-going care with community agencies; and developing and updating patient care plans. Assist with organizing services across provider lines, between people and systems to affect optimal patient outcomes, achieve continuity of care, and reduce costs. Serve as a utilization management resource to determine the appropriateness of admission, level of care, and continued stay. Assist in evaluating and collaborating with internal partners to assess patient needs through intake, care gap identification, screening for social drivers/ determinants of health (SDOH), and providing triage if needed. Provide education related to the resources available and recommend and/or facilitate the care plan/service plan and/ or strategies to identify and address social drivers of health (SDOH) and enhance patient well-being.

Skills, Knowledge, & Experience Requirements:
- Graduated from an accredited School of nursing is required.
- Current RN license in the state of Oregon is preferred.
- Two years of experience as a Resident Care Manager in a long-term care facility is preferred.
- Good working knowledge of long-term care, and state and federal regulations/requirements is required.
- Knowledge of procedure authorization and its direct impact on the practice’s revenue cycle.
- Understanding of payer medical policy guidelines while utilizing these guidelines to manage authorizations effectively.
- Basic understanding of human anatomy, specifically musculoskeletal.
- Proficient use of CPT and ICD-10 codes.
- EPIC experience a plus.
- Ability to function as a team member and communicate effectively is required.
- Strong interpersonal skills with a drive to provide exceptional service.
- Ability to maintain strict confidentiality.
- Attention to detail and organizational skills.
- Excellent oral and written communication, and computer proficiency specifically, in Microsoft Office and online software.
