Starting Pay: $19.90 - 22.05/hr
Sponsors Purpose
Sponsors seeks to improve outcomes and opportunities for individuals with conviction histories and to assist individuals releasing from incarceration in their successful reintegration to our community. Sponsors is nationally recognized as a model utilizing best practice in reentry services.

Position Reporting
This position reports to the Transitions Program Director.
This position does not have supervisory responsibilities.
Job Responsibilities
Oversee all aspects of Case Management for assigned program participants.
Skills and Knowledge:
Successful candidates for this position must not be on parole or probation, and cannot be in a diversion program (e.g. SB416, Treatment Court, DUII Diversion, Restorative Justice).
Employment is contingent on a background clearance by the Oregon Youth Authority through their external review and approval process.

Job Responsibilities
Oversee all aspects of Case Management for assigned program participants.
- Perform screening and intake for incoming residents using program-approved assessment tools.
- Monitor status of individualized case plans based on LS/CMI transitional assessment and/or reassessment as needed.
- Initiate referrals; coordinate and communicate regularly with participants’ parole officers and treatment providers.
- Monitor compliance with conditions of each participant’s release and all Sponsors program requirements.
- Monitor compliance with requirements of other related programs, including cognitive groups, drug/alcohol treatment, parenting classes, and others as applicable.
- Conduct regularly scheduled 1:1 sessions with each participant and maintain communication as necessary.
- Perform scheduled and random drug and alcohol screenings
- Utilize web-based programs and other systems to chart participant progress, maintain accurate entry/discharge records, and coordinate care.
- Facilitate (as needed) Sponsors cognitive behavior intervention groups, ensure participant compliance, and maintain all group related records in the agency designated time frame.
- Assist participants in developing transition plans with a goal toward long-term stable housing that meets the approval of assigned parole officers.
- Monitor and discuss participants’ employment, savings, and financial management, particularly as it relates to their transition from Sponsors and into stable housing.
- Maintain appropriate participant records, both electronic and physical, as required by Sponsors, collaborating agencies, and grant requirements.
- Monitor common areas and individual housing units to ensure the safety, security, and cleanliness of the facility.
- Receive and document physical donations (e.g., clothing items).
- Receive and route calls, faxes, and walk-in program visitors.
- Attend and actively participate in scheduled program staff meetings, biweekly all-staff meetings, and weekly case management meetings.
- Participate in scheduled trainings as requested by management staff or Transitions Program leadership.
- Maintain regular direct communication, both written and verbal, with program supervisors and other Sponsors staff, in addition to community partners regarding participant progress and conduct, facilities operations, and organizational protocols/procedures.
- Perform other related duties as assigned by treatment team or Mentorship Program Director
Skills and Knowledge:

- Ability to prioritize tasks and to delegate them when appropriate

- Proficient with Microsoft Office Suite or related software
- Ability to work effectively with a wide range of people to achieve individual and organizational objectives
- Committed to the mission and values of Sponsors and reentry services
- 2 years of experience providing direct client services; OR
- 1 year providing social services to individuals in the criminal legal system
- Preference for case management experience
- Higher education in a related field preferred
- The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties
- This position may travel between locations at the direction of the supervisor in an effort to meet program needs
- May encounter frequent interruptions throughout the workday
- May encounter individuals experiencing distress
- Walking up and down stairs
- Sitting at a desk for prolong periods of time
- Noise level in the work environment is typical of that of an office environment
