Overview:
The Case Manager/Care Coordinator Level 2 plays a vital role in connecting consumers with essential services, including behavioral health, medical, social, educational, and other health-related resources. This position prioritizes consumer advocacy, effective care coordination, and the development of person-centered treatment plans.

Qualifications:
- Bachelor’s or Master’s degree in a human services-related field, approved by the West Virginia Board of Social Work
- OR a licensed Psychologist with a Master’s or Doctoral degree
- OR a licensed Registered Nurse
- Valid driver’s license and satisfactory driving record
