About the Role
The Home Health Patient Care Manager is a critical position that requires strong leadership skills and experience in coordinating clinical services. As a member of our Southern Home Health team, you will work closely with our interdisciplinary staff to provide high-quality care to our patients.
Your primary responsibility will be to oversee the supervision and coordination of clinical services, ensuring that our patients receive the best possible care. This includes coordinating with physicians, clinicians, and other healthcare providers to develop and implement individualized plans of care.
- You will be responsible for ensuring that patient needs are continually assessed and care rendered is individualized to patient needs, appropriate, and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- You will oversee and assure development, implementation, and updates to the individualized patient plan of care, as appropriate.
- You will manage and document phone calls from physicians, clinicians, patients, referral sources, and communicate patient updates/new orders to clinicians. You will use coordination notes to document, as needed and appropriate.
- You will review assessments and plans of care daily, per assigned workflow, and consult clinicians with recommendations, as appropriate.
- You will coordinate all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
