THIS IS A FULL TIME POSITION IN A CLINIC ENVIRONMENT (MOSTLY DESK WORK)
MONDAY-THURSDAY 8-5 AND FRIDAY 8-3 Salary DOE
Job Summary```
As a Nurse Navigator, you will play a crucial role in assisting patients with their healthcare journey. You will provide patient assessment, health coaching, and support to the providers, research team and patients to ensure optimal care and outcomes.

Duties```
- Conduct patient assessments and develop individualized care plans
- Provide health coaching and education to patients and their families
- Coordinate and facilitate communication between patients and healthcare providers
- Assist patients in navigating the healthcare system, including scheduling appointments and coordinating referrals
- Collaborate with interdisciplinary teams to ensure continuity of care
- Monitor patient progress and outcomes, making adjustments to care plans as needed
- Utilize electronic medical record systems to document patient information accurately
