What are the responsibilities and job description for the MOBILE CRISIS CASE MANAGER position at Southside Behavioral Health?

Covering the counties of Halifax, Mecklenburg and Brunswick, VA. our behavioral health services are supported by community involvement and professional values.
Provides crisis intervention, crisis stabilization, and referrals to needed clinical services to clients in the community. This position is based in the clinician’s home, with immediate response to the community when dispatched by the Crisis Hub and a minimum of weekly onsite check-ins at the SBH Response & Recovery Center. Provides services within 1 hour of home base and is required to respond within 1 hour of dispatch. Residency in Halifax County, Mecklenburg County, or Brunswick County required. Works as part of the Regional Mobile Crisis team to assure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community in the least restrictive environment. Works closely with the Regional Mobile Crisis team, Child and Family Services Department, Outpatient Services Emergency Services and REACH. Knowledge and experience with working with individuals experiencing behavioral health crises and techniques and interventions to effectively de-escalate crisis situations required.
Responsibilities
- Writes effective and measurable individualized service plans as required upon admission.
- Provides care coordination to individuals in crisis. Assists with crisis resolution and generalized problem-solving. Provides crisis support as assigned.
- Collaborates with SBH departments and programs to ensure that services reflect the client’s needs. Will complete referrals for needed services on a timely basis.

- Promotes independence by teaching and modeling life skills, which may include personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem, and self-reliance.
Qualifications
- Bachelor’s Degree in Human Services, Counseling, Psychology, Social Work or RN required.
- Must be approved and registered with the Virginia Board of Counseling as a QMHP or registered as a QMHP-Trainee within 6 months of hire.
- Must have, or be eligible to obtain, certification as a pre-screener for emergency hospitalization in Virginia within six months of hire.
- Must meet all criminal background and drug screening requirements.
- Valid driver’s license and safe driving record, required.
