About the Role
As a Referral Specialist, you will play a vital role in ensuring seamless patient referrals to external healthcare specialists while providing exceptional support and advocacy for patients navigating the healthcare system.
Key Responsibilities
- Accurately register patients and verify demographic and insurance information.
- Advocate on behalf of patients to ensure timely approval of medical equipment and services.
- Assemble clinical information for patient referrals and coordinate with external providers.
- Inform patients about referrals, documentation, and appointment details.
- Ensure timely processing of referrals, including follow-ups for missed appointments.
- Manage internal urgent vision referrals and community health worker services referrals.
- Participate in the SAGE SCOPE program and maintain a positive relationship with program staff.
- Support patients with transportation, interpretation services, and navigating the healthcare system.
- Document and track referrals to ensure quality care and outcomes.

Qualifications:
- High school diploma required.
- Experience as a Medical Assistant or knowledge of medical terminology preferred.
- PPX (Lattice Pro) experience a plus.
- Bilingual (English/Spanish) required.
