Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and effective care throughout their healthcare journey. This position involves working on a team with a master level therapist to provide care for mentally ill consumers, support to their families and to facilitate communication, coordinate services, and manage care plans in the consumers home environment. The ideal candidate will possess a strong understanding of mental health and accessing support services within a home, community and office setting.
Duties
- Serve as a team partner, point of contact for patients, address inquiries and provide information regarding services and care options.
- Coordinate appointments, referrals, and follow-up care to ensure seamless patient experiences as part of a two person team.
- Maintain accurate patient records and documentation in compliance with healthcare regulations.
- Collaborate with therapist and other healthcare professionals to develop and implement individualized care plans.
- Assist in managing the flow of information between patients, families, and healthcare providers.
- Educate patients about their treatment options, medications, and self-care strategies.
- Ensure that all administrative tasks are completed efficiently within the home or community/office environment.
Requirements
- Bachelor degree required, in human service related field preferred.
- Proven experience in mental health care support within the home or community setting is preferred.
- Familiarity with mental health terminology to effectively communicate with healthcare professionals and consumers.
- Strong organizational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication skills, both verbal and written, to facilitate clear interactions with consumers and staff.
- Proficiency in documenting in electronic health record systems is a plus.
- Ability to work collaboratively as part of a team while also being self-motivated.
- A compassionate approach to consumer care with a focus on enhancing consumer satisfaction and increased well being.
