Southwest Colorado Community Corrections Center has a long history of supporting offenders transitioning through the criminal justice system in Durango. We are a 54 bed residential facility and offer on-site substance abuse testing and case management services to residents and clients in the community. CASE MANAGER
Responsibilities:
Provides case management services to clients to assist them in preparing for eventual release to independent responsible community living and/or monitor progress for those recently transitioning into community living. All employees are expected to conduct themselves in a professional manner and to maintain a high ethical standard at all times.
Duties include:
- Complete intakes using Standardized Offender Assessment Revised (SOAR) and orientation for all new clients.
- Implementation of best practices
- Work together with clients to compile a Progression Matrix that helps facilitate change by designing a case plan that addresses a client’s criminogenic risk factors and builds upon their stability and positive behavioral factors.
- Actively monitor client performance and adherence to provisions of the Progression Matrix. Including, but not limited to, Progress on criminogenic needs, stages of change, stability factors, case plan action steps, behavior progress and disciplinary reports.
- Discuss client progress in routine case rounds (staffings), identifying areas of concern and provide reasoned recommendations for program case plan modification.
- Maintain all client case management records.
- Work within the guidelines set by Community Corrections Standards
- Assist other case managers in management of their cases when those case managers are off duty.
- Serve as a positive role model for all clients by imparting professional dress and behavior.
- Interface with various community service organizations in order to assist in providing clients with the highest quality of service possible.
- Maintain an active case load not to exceed 20 clients
- Interface with other staff and volunteers to ensure consistent service to all clients.
- Conduct budget planning sessions with assigned clients and implement same.
- Attend and participate in conferencing, training programs and seminars
- Assume all case management responsibilities when no case manager is on duty.
- Attendance at monthly treatment and weekly Hilltop staffing.
- Maintain a professional demeanor and act in an ethical manner at all times.
- Other duties as assigned.
Minimum Qualifications: Bachelors Degree or equivalent, experience in human
services field with emphasis in guidance and counseling
or equivalent areas.
CPR and First Aid certification, ability to work weekends, evenings, and graveyards.
