SUMMARY : The Licensed Practical Nurse works collaboratively with providers and support staff to ensure the smooth and efficient running of the Internal Medicine Department.SPECIFIC DUTIES AND RESPONSIBILITIES : All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).
- Processes all patients according to the policies and guidelines established for the Internal Medicine Department.
- Ensures continuity of care is maintained in all aspects of patient care.
- Responsible for data collection systems and to report statistical data necessary for grant purposes as needed.
- Supports all quality assurance activities and infection control procedures according to established policies.
- Assists in maintaining stock inventory and ordering supplies, as necessary.
- Attends department, case management and other meetings as assigned.
- Maintains strict confidentiality at all times.
- Performs other duties and tasks as assigned.Qualifications :
- Holds a current Connecticut Practical Nurse license
- Experienced and able to function effectively and professionally within an outpatient setting providing services to a special and diverse population groups.Education and / or Experience :
