Migrant Health Outreach Worker
Job Summary
Southwest Virginia Community Health Systems is an organization made up of Federally Qualified Health Centers that serves the area of rural Southwest Virginia. We have been serving patients for over 40 years and are currently seeking a Migrant Health Network Outreach Worker to join our team working in Washington/Smyth Counties, VA.
Southwest Virginia Community Health Systems (SVCHS) consists of a group of non-profit Community Healthcare Centers, a Migrant Health Network, an Integrative Behavioral Healthcare Program, a Substance Abuse Recovery Program, and the Southwest Virginia Regional Dental Center. SVCHS was established to improve access to healthcare services by providing comprehensive, patient-centered care to the communities we serve.

- Serve as intermediary between the community health centers and farm workers. Work independently to located farm workers and provide case management, health education, transportation, and translation services. Maintain positive relationships with growers, farm workers, and clinic staff. Must document all activities as performed.
Outreach
– Locate farm workers throughout the jurisdiction to which assigned and complete health assessments at each location.
– Inform farm workers of clinic services and other community services.
– Compile jurisdiction information (grower list, community agency contacts, etc.) in Continuity File to assist future staff.
- Case Management
– Interview farm workers to determine needed services.
– Provide case management by assisting farm worker to obtain needed services in community through referrals, phone calls, and interpretation, translation, and advocacy activities.
– Seek methods of transportation and translation to assist farm worker to obtain needed assistance. These services may be provided by outreach worker if other resources are not available.
- Health Education
– Provide health education at farm worker location or in clinic setting.
– Teach or organize pre-natal sessions as needed.
Qualifications
– Must have Qualified Medical Interpreter or Bridging the Gap certificate or equivalent.
- Three (3) years of experience working with social services programs.
- Extensive knowledge of program development and implantation.
- Thorough knowledge of state and local social service resources.
- Thorough knowledge of Medicaid reimbursement procedures and awareness of other third-party reimbursements.
- Thorough knowledge of medical record requirements, social service standards and staff training resources.
- Ability to establish rapport and relate to clients, families, community agencies and groups, and variety of service disciplines.
- A working knowledge or understanding of substance abuse issues.
- Must be qualified in Basic Life Support techniques and CPR.
- Must have a valid driver’s license.
- Must be a U.S. citizen
- Must be bilingual (Spanish/English)
- Must have good driving record.
