What are the responsibilities and job description for the SHP Housing Support Specialist position at Southwestern Community Services-Head Start?
Job Summary:
Provides housing services and appropriate supportive housing for SHP participants, develops programs and resources, and formulates case plans that promote moving towards self-sufficiency. Ensures the safety of all residents by implementing, monitoring, and enforcing the rules and regulations of tenant living and coordinate the maintenance of the buildings and grounds. This position may have a flexible schedule and remote options when necessary.
Essential Functions:
· Completes a needs assessment and acts upon critical needs appropriately and immediately
· Orients clients to the Supportive Housing Program's rules and regulations
· Develops a comprehensive client-driven case plan with both short-term and long-term goals identified
· Completes a timeline and measures for each goal
· Monitors progress towards goals in regularly scheduled sessions
· Evaluates and adjusts case plans as needed
· Empowers clients to become involved in their own planning and goal setting
· Refers clients to appropriate resources to assist with meeting goals
· Assures that specific HUD program goals are consistently being met and provides extensive case work to foster transition from homelessness to permanent housing
· Refers Clients to Financial Literacy, Tenancy 101, and other training programs aimed at learning and practicing life skills and decision making
· Assesses the program for effectiveness and producing behavioral changed in clients
· Participates in Crisis Intervention/Prevention training, HIPAA and Bloodborne Pathogen training and other trainings deemed necessary to the position
· Maintains client files to include conversation, warnings, progress toward goals and documentation of any incidents
· Reports critical incidents immediately to the Supportive Housing Program Manager.
· Collects data necessary to meet funding requirements and statistical reports
· Assures the safety of each property through frequent tours of the facilities inspecting for any hazards or repair needs
· Reports any hazards to the Sullivan County HSS Manager for immediate attention to ensure safety for the residents
· Facilitates the repair process in collaboration with the Sullivan County HSS Manager
· Completes annual recertifications with clients
· Maintains ongoing communication with the Supportive Housing Program Manager
· Collaborates with teammates and other agency staff as appropriate

Desired Qualifications:
· Associate’s degree
· Bachelor’s degree preferred
· Or equivalent in education and experience
· Human Services experience
· Customer Service experience
· Some Management or Supervisory experience
· Experience with disabilities helpful
· Valid NH Driver's license and vehicle with liability insurance

Pay: From $18.00 per hour
