Job Summary:
The VA (Veterans Administration) Coordinator works efficiently, directs, and coordinates HHA (Home Health Aide) They are responsible for overall program operations, including compliance with rules and regulations as established by the Veterans Administration. Additionally, the Coordinator will provide support services and coordinate with other healthcare providers to ensure that clients receive the best possible care. Responsibility includes supervision and staffing of HHA’s, monitoring clients’ progress, reviewing care plans, and submission of Request for Services Form (RFS) as needed.
Requirements & Qualifications:
- Prior Military service a plus.
- Degree in Social Work, Licensed Practical Nurse, or related field preferred.
- A minimum of two years’ experience in health care or related field.
- Strong organizational, communication, and problem-solving skills.
- Ability to work independently and multitask.
- Able to build and maintain strong relationships with clients, families, and other personnel.
- Knowledge of applicable regulations.
- Able to work flexible hours if needed.
- Demonstrate tact and diplomacy skills.
- Serves as the point of contact for patient inquiries and complaints.
Duties and Responsibilities:
- Implement home care plans for clients via VA Authorization.
- Monitor client progress and communicate with the Veterans Administration representative to make necessary adjustments to care plans.
- Answers inquiries, counsel’s patients on VA guidelines and program home care benefits.
- Coordinate with other healthcare professionals to ensure clients receive the best possible care.
- Prepares, executes, and tracks required reporting information.
- Reviews and interprets VA correspondence.
- Maintain accurate and up-to-date records of client authorization plans.
- Provide support and guidance to family members and caregivers.
- Ensure compliance with all applicable regulations and laws.
- Provide emotional support to clients and their families.
- Participates in on-call duties.
- Assist in marketing and outreach efforts.
- May travel to several NM counties.
- Overnight travel may be required on an infrequent basis.

Pay: From $21.00 per hour
Expected hours: 40 per week
