Sovereign Home Health is looking for an LPN with self determination and organized skills to see home bound patients, providing skilled interventions such as wound care, blood draws, med setup, health status evaluations, ADL assistance, and a wide variety of other nursing skills. This position is a part time position with hours ranging between 24 and 30 per week. There are benefits available with ETO and sick time accrual, as well.
Summary
As a Part-time LPN Home Health Nurse, you will provide essential nursing care to patients in their homes, focusing on wound care and overall patient well-being. Reporting to the Home Health Supervisor, you will utilize your core skills in patient assessment, catheterization, and basic life support to deliver high-quality care. Your premium skills in IV infusion and experience in long-term care will enhance patient outcomes, while your relevant skills in home and community care will ensure a compassionate approach to nursing. Responsibilities
- Provide skilled nursing care to patients in their homes, including IV insertion and wound care management.
- Conduct thorough patient assessments to monitor health status and develop care plans.
- Administer medications and treatments as prescribed, ensuring adherence to HIPAA regulations.
- Collaborate with healthcare teams to optimize patient outcomes and provide education to patients and families.
- Maintain accurate electronic health records (EHR) and documentation of patient care activities.
