Responsibilities
- Provide direct patient care, including wound care and catheterization, while adhering to HIPAA regulations.
- Conduct thorough patient assessments and document findings in EHR systems.
- Administer medications and IV infusions as prescribed, ensuring patient safety.
- Collaborate with healthcare teams to develop and implement care plans.
- Educate patients and families on health management and care procedures.
- Maintain a clean and safe environment for patients in home health settings.
Qualifications
- Valid LPN license and certification in Basic Life Support (BLS)
- Proficient in patient care, assessment, and catheterization
- Experience with EHR systems, particularly Epic or Cerner
- Knowledge of wound care and physiology
- Familiarity with HIPAA regulations and patient confidentiality
- Experience in home health, long-term care, or assisted living settings
- Ability to perform tube feeding and IV infusion as needed
