Job purpose
Our Direct Coordinator position works with individuals to provide home based care and services on their behalf, as well as contributes to the company and management team with the goal of improving and maintaining high-quality performance standards and reputations within the communities we serve. This position requires a person with energy, creativity and desire for improving the lives of others. Maintain the goal of delivering desirable outcomes to our clients while maintaining our company’s compliance.
Duties and Responsibilities
- Partake in admission meetings and fulfil documentation requirements.
- Ability to write, understand and implement a variety of Community Support Plans and Individual Service Plans per individual outcomes and goals.
- Have knowledge in current HCBS licensing standards/requirements to ensure Sparkles Home Health Care, LLC compliance with the programs they are responsible for.
- Review and ensure compliance with each person served with all 245D paperwork including Coordinated Support and Service Plan (CSSP), Coordinated Support and Service Plan Addendum (CSSPA), Individual Abuse Prevention Plan (IAPP), and Self-Monitoring Assessment (SMA).
- Working a minimal of 16 hours of direct care per week in the community with client.
- Recruit, Train and schedule trainings for the support staff. Assist with scheduling, including alternating on-call duties
- Monitor/Evaluate documentation daily and provide/schedule training to staff as needed to ensure consistency and compliance in documentation.
- Provide supervision to the direct support staff(s) which includes monthly meetings, annual reviews, maintaining employee files/notes stay compliant, and corrective action when necessary.
- Ability to outrage and build relationship with case management as well as adding new clients.
- Take part in progress evaluation meetings on effectiveness of service delivery, methodologies, and progress on the person’ outcomes based on the measurable and observable criteria. Ensure that outcomes are appropriate, and person centered.
- Review evaluation for each person of positive support strategies and Person-centered plans that lead to the success of the person.
- Coordinate monthly staff meetings in which policies and procedures are reviewed including office safety with monthly safety drills (if required), site trainings and any additional training as situations indicate a need for them.
- Other duties as assigned and agreed upon.
- Perform other duties as assigned, contributing to the overall success of the team and the individuals' receiving services, while working in partnership with leadership to achieve the team's goals.
REQUIREMENTS:
To qualify for the position, applicants must:
- A bachelor's (BA) degree in a field related to human services and 1 year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 or older
- OR an associate (AA) degree in a field related to human services and 2 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older
- OR a diploma in a field related to human services from an accredited post-secondary institution and 3 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older
- OR a minimum of 50 hours of education and training related to human services and disabilities and 4 years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older under the supervision of a staff person who meets the qualifications identified in 1 to 3 above
- Must be mature, polite, and professional
- Must possess effective listening skills and strong overall social and communication skills
- Department of Human Services background clearance
- Valid driver's license, reliable vehicle, acceptable driving record, and proof of auto insurance required
- Ability to effectively communicate in English, both written and verbally
- Strong 245D Familiarity
