Summary
Under the direction and supervision of the ISP Program Manager, the Inside Safe Program Supervisor will be responsible for providing leadership, mentoring, training and program quality assurance to our case management team, to ensure the highest quality provision of services. Program Supervisor’s primary responsibilities will be to conduct daily tracking of case management activities, be responsible for day-to-day oversight of field operations, develop and coordinate relationships with partner agencies, ensure program quality control and documentation accuracy, provide ongoing training to the team, and monitor follow-through of client enrollments, linkages and referrals. The Program Supervisor will determine the level of supportive services each individual and families being served will need. The Program Supervisor will support the staff in understanding and completing the level of services needed for the programs including Motel support.
Essential Functions
  • Must know, understand, and be able to articulate the mission, vision, and core values of HOPICS
  • Establish a working knowledge of all HOPICS programs and their eligibility criteria for referral and linkage purposes
  • Ability to understand and carry out oral and written directions.
  • Assist in creating Client services plans, notes, assessments and linkages
  • Understand the landscape of homeless services in SPA6
  • Conduct outreach in the field with the multi-disciplinary treatment teams
  • Complete CES packets
  • The Supervisor will supervise client files and ensure adherence to LAHSA definitions and guidelines; convene and facilitate weekly staff meetings; attend all project meetings and training courses; implement and monitor SSG personnel policies and procedures for compliance with applicable state and federal regulations; coordinate staff development and training. 
  • The supervisor will provide assignments and assist with the development of systems for administrative staff; communicate effectively with the contract monitor and funder; and work with fiscal and human resources personnel. Effectively communicate with Manager, Associate Director and/or Division Director and partner agencies
  • Implementing LAHSA imposed HMIS policy
  • Work collaboratively with program staff to address any barriers such as financial hardships, mental health, or physical health issues that may prevent the individual from successfully maintaining permanent housing.
  • Monitor staff outreach efforts to ensure high quality service provision
  • Track daily staff outreach activities and report summaries to the Inside Safe Program Manager
  • Assist in weekly reporting to HOPICS management
  • Establish and maintain working relationships with partner agencies, with housing providers, mental health, substance use and primary care providers, law enforcement agencies, benefits programs, employment programs, and other supportive service providers in SPA6
  • Monitor follow-through for clients linked to services, including mental health, substance use, primary care, housing and benefits programs to ensure clients are receiving services and to assist them in system navigation to achieve housing stability
  • Provide quality control duties to ensure the required documentation in client charts is accurate and up to date
  • Assist Inside Safe Program Manager in program quality improvement projects
  • Attend daily Inside Safe team “huddles” and weekly programming meetings
  • Work closely with the SPA6 Outreach Coordinators to plan daily outreach activities
  • Provide ongoing training and support to all the Inside Safe teams
  • Develop training materials to ensure proper and ethical outreach by all Inside Safe staff
  • Attend division wide meetings and training as needed or required.
  • Attend all programs, departments, and funding specific meetings and training as required or needed.
  • Facilitate internal referrals and/or appointments within HOPICS and coordinate services with other HOPICS programs.
  • Perform any other appropriate responsibilities as assigned by the Inside Safe Program Manager, Senior Manager for Community Services & Division Director.
  • Maintain accurate files/records on activities, client services in compliance with HIPAA, CFR 42 and other funding requirements for auditing purposes.
  • Attend meetings associated with other projects as assigned by the Inside Safe Program Manager, the Senior Manager for Community Services or the Division Director
  • Maintain appropriate boundaries with staff, clients and community partners.
  • Adhere to SSG’s Code of Ethics and HOPICS’ Core Values.
  • Represent the Agency in a professional manner at meetings and community events.
  • Responsible for coordinating van usage, van maintenance and upkeep
  • Identify knowledge gaps and provide appropriate ongoing outreach training to address staff training deficits
  • In conjunction with the Program Manager, assist in oversight and supervision of Inside Safe Case Managers.
  • Maintain relationships with funders, county, landlord relationships, and other initiative efforts contact representatives.
  • Work collaboratively with funder staff and community stakeholders; attend community and stakeholder meetings as required.
  • Provide guidance and support to subcontractors.
  • Coordinate with the housing, quality assurance, and fiscal departments to ensure documentation completeness and financial assistance disbursements.
  • Efficiently manage and monitor data integrity and quality assurance of client and program milestones and service outcomes.
  • Conduct weekly client support meetings with housing navigators and case managers.
  • Conduct a 1:1 meeting with staff for weekly supervision.
  • Complete program budgets as needed.
  • Ensure that special projects are allied with outreach, housing stabilization, and permanent housing.
  • Monitoring program budgets monthly
  • Oversee weekly client care meetings with motel support case managers.
  • Oversee project management for Motel clients, and ensure invoices are submitted monthly.
  • Oversee all special projects efforts.
  • Conduct staff filing audits and reviewing HMIS entries.
  • Regular attendance required
  • Other duties as assigned

Minimum Qualifications - Knowledge, Skills and Abilities Required
  • Bachelor’s degree from an accredited university OR
  • Two years’ experience working in the social service field
  • If in recovery, a minimum of three (3) years of being drug and alcohol free is MANDATORY.
  • Ability to work with clients from a diverse cultural, ethnic, and socio-economic background.
  • Verification of Employment Eligibility and Background Clearance.
  • Ability to communicate effectively, both written and orally.
  • Valid driver’s license and reliable transportation.
  • Current proof of Auto Insurance is required.
  • TB test required (Not more than (3) months prior to or (7) days after date of hire and renewal annually thereafter.
  • CPR and First Aid Certification required within 30 days of employment with the company.
  • Basic computer word processing skills required.
  • Ability to understand and carry out oral and written directions.

Supervisory Responsibilities:
  • The Program Supervisor will be responsible for supervision of Case Management teams and may be responsible for other staff as needed.  The Program Supervisor will be responsible for approving staff timesheets, disciplinary actions, staff evaluations and other duties associated with administrative supervision as necessary.


Environmental Conditions (Working Conditions)
  • The environment for this position is office and field based.   Must be able to work in an environment with many priorities, busy, and fast paced.   Must be able to solve problem solve, adapt to changes that are unpredictable.  Must be able to work with highly aggressive clients. Must be able to work in various shelter environments.
  • Exposed to highly aggressive clients: must communicate too many sources including courts and outside community agencies: sometimes noisy, loud and disruptive clients.

Physical Requirements
  • The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or control; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb, balance, or stoop; rarely to crouch or crawl. The position also requires driving of personnel and company vehicles.
 
  • The employee must occasionally lift and / or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.

  • This position will require the individual to be able to handle any/all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules: and be able to process information, think, and conceptualize.
