Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services across the continuum of care for select members to promote quality, cost effective hospice care. The ideal candidate MUST live in MS.

Work Hours/Shift:
Full Time Monday – Friday 8:00am - 4:30pm with occasional on call responsibilities
Education
Graduate from NLN school of nursing
License
Current Mississippi State licensure
Experience
Hold RN license in MS. Must have at least one year experience in an acute care setting. Preferably experience in hospice or home health. Reliable transportation and willing to drive.
You Will Be Responsible For:
- Manage a caseload of hospice patients within our north MS territory- prefers candidate that lives in MS.
- Demonstrate compassion and understanding of end-of-life care and hospice philosophy
- Drive and travel to care for patient/family in their places of residence
- Assess and reassess patients per hospice policy
- Develop, implement, and evaluate the plan of care (IPOC)
- Able to work collaboratively with IDG, physician, referral sources and nursing facilities
- Manage workload and prioritize care
- Administer medications as prescribed
- Facilitate discharge, transfers, and revocations per policy
- Provide patient, family, and/or significant other health teaching/education
- Communicate/collaborate with others in providing patient care
- Provide clinical leadership
- Utilize critical thinking and nursing judgment skills
