What are the responsibilities and job description for the Case Manager/Clinician position at Spectrum Youth & Family Services?
Are you passionate about empowering youth and making a meaningful difference in your community?
Join our Compass Program team as a Compass Clinician! In this role, you'll work with at-risk youth (ages 12-23) and their families, providing critical services like assessment, case planning, crisis response, and family preservation. As part of a dedicated team, you'll help youth achieve their goals through strengths-based treatment planning and comprehensive case management, focusing on areas such as housing, education, employment, mental health, and substance use support.
The Compass Clinician carries a caseload of 14-16 youth and families, offering direct services and support on a weekly basis. You’ll develop individualized treatment plans that leverage client strengths, conduct assessments to identify risks and needs, and connect youth to vital resources, including housing, education, employment, healthcare, and community support. Collaboration is key in this role—you’ll work closely with families, other clinical staff, and community partners to stabilize and strengthen family systems. This position also includes participation in a monthly on-call rotation to provide emergency phone intervention as needed. Additionally, the role involves timely and accurate documentation, outreach to local resources, and educating referral sources about our services. This position is located in St. Albans, VT.
