Job Summary:
 
The Housing Outreach Specialist will be instrumental in reaching out to individuals experiencing homelessness. This individual will provide information to these individuals on area resources available to them.  This position will provide Outreach, General Case Management, and  Housing Transition to eligible participants assessing and connecting them with appropriate services.  The specialist will complete appropriate assessment activities, coordinate needed evaluations and diagnosis assessments, manage and implement treatment planning.  The Housing Outreach Specialist will also utilize the grant direct assistance funds as appropriate and authorized. 

The Housing outreach specialist role requires strong interpersonal and communication skills, empathy and a comprehensives understanding of housing resources and eligibility criteria. The person who holds this position must have a philosophy of promoting and fostering housing stability for the participants. This individual will demonstrate healthy and supportive interactions with participants that models a positive recovery model perspective that is also culturally sensitive and respectful.

Duties/Responsibilities:
 
  • Conduct outreach activities to identify individuals in the community experiencing housing instability.
  • Engage with individuals and families to assess their housing needs and provide support and guidance in accessing housing stabilization services. 
  • Track and provide, compile, and enter needed data including entry into the Coordinated Entry system. 
  • Utilize the Homeless Management Information System (HMIS) tool. 
  • Collaborate with community partners, including homeless shelters, housing authorities, social service agencies, and healthcare providers, to facilitate referrals and enhance service coordination.
  • Maintain thorough documentation of outreach activities, client interactions, referrals made, and outcomes achieved.
  • Participate in community events, forums, and meetings to advocate for individuals experiencing housing instability and promote our services.
  • Attend bi-monthly case reviews and clinical supervision.
  • Assist with the planning and coordination of Rent Wise classes.
  • Communicating and interacting effectively and respectfully with employees, supervisors, individuals from other organizations to promote health and professional working relationships.
  • The Housing Outreach Specialist will attend regular meetings with the provider agencies including, but not limited to the Continuum of Care, Local Mental Health Authority, Counties, local HRA’s and other partners in the community to support coordination and advocacy with these providers. 

Required Knowledge, Skills, and Abilities:

  • Ability to manage confidential information responsibly.
  • Ability to listen, comprehend, and effectively communicate information both in writing and orally with all individuals.
  • Knowledge of agency and departmental regulations and policies

Education and Experience:
 
  1.            Must pass required Applicant Background Check
  2.            21 years of age or above.
  3.            Have a valid driver’s license.
  4.            Has a High School diploma/GED And, have at least one of the following:
  • A course of study in a health or human services-related field leading to a Bachelor of Arts, Bachelor of Science, or associate degree; or 
  • Two years of experience with the target population served (serious mental illness, homelessness)
 
Physical Requirements:

  • Requires incumbent to work inside, outside, alone, with others, around others, and have contact with the public.
  • Activities that can occur include but are not limited to the following: sitting, climbing staircase, standing, walking, depth perception and visual accommodation. They also include bending, stooping, pushing, pulling, twisting, reaching, some lifting and carrying. 
Work Environment:
 
The employee must be willing to transport clients in their own vehicles, work in clients’ home appropriately, and interact with clients in the community. Most duties are performed in an office or public-school setting. Provide services in-person and via telehealth.
