Key Responsibilities:Patient-Focused Home Health Therapy

• Provide one-on-one in-home physical therapy services, following the care plan established by the Physical Therapist (PT).
• Assist patients in regaining mobility, strength, and independence through therapeutic exercises and functional training.
• Educate patients and caregivers on home exercise programs and safe movement techniques to optimize recovery.

Collaboration & Care Coordination

• Work closely with Physical Therapists, physicians, nurses, and other healthcare professionals to ensure the highest quality of patient care.
• Participate in patient case conferences, interdisciplinary meetings, and quality assurance programs (QAPI).
• Maintain accurate, up-to-date documentation in compliance with Medicare and home health regulations.

Compliance & Professionalism

• Adhere to all state, federal, and Medicare guidelines for home health therapy services.
• Provide compassionate, evidence-based therapy interventions while ensuring patient safety.
• Uphold the mission, vision, and values of Spirit Homecare & Hospice in every patient interaction.

Who We’re Looking For:

✔ Licensed Physical Therapy Assistant (LPTA) in Ohio (or eligible for licensure).
✔ 1-2 years of experience preferred in home health, outpatient therapy, or skilled nursing facilities (new grads with strong clinical skills will be considered).
✔ Excellent clinical judgment and communication skills—able to work independently while collaborating with the team.
✔ Compassionate, patient-focused professional with a strong work ethic and attention to detail.
✔ Reliable transportation, valid driver’s license, and auto insurance (travel is required for home visits).

Work Environment & Schedule:

✔ Schedule: Monday – Friday (Full-Time & PRN options available)
✔ Work Location: In-home patient visits in West Chester Township, OH, and surrounding areas
✔ Travel Required: Reliable transportation is a must for travel to patient homes.
