Registered Nurse (RN) Case Manager - Monday thru Friday, On Site

**The position is on site at the facility, and remote work will not be considered.**

Spring Creek Rehab and Nursing Center is currently looking for a Full Time Registered Nurse (RN) Case Manager to assist with leading our nursing team! **The position is on site at the facility, and remote work will not be considered.** Are you a compassionate and enthusiastic Registered Nurse (RN) with case management experience in long-term care? Requirements of the Registered Nurse (RN) Case Manager:

  • Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) in this state
  • Bachelors Degree in Nursing (BSN) from an accredited college or university preferred.
  • Must have, as a minimum, two (2) years of experience with case management skills in a long-term care setting.
  • Proficient in ability to interpret Managed Care/Medicare Advantage Contracts and provide education to the teams in our centers.
  • Knowledge of the various payment models for Managed Care
  • Must have Managed Care/Medicare Advantage billing knowledge and experience.
  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilites of the Registered Nurse (RN) Case Manager:

  • Provides communication to the managed care providers regarding a patient’s care and their current level of functioning.
  • Ensures comprehensive care is continued from hospital to skilled nursing center
  • Insurance updates-with focus on increase in levels and RUGs when appropriate
  • Assists in gathering and reviewing patient records for appeal requests
  • Medicare Certifications/Recertifications
  • Assisting with querying the physician for appropriate diagnosis codes for best possible reimbursement
  • Assist in reviewing hospital records for diagnosis codes, treatments and medications.
  • Interviews of the patient when appropriate
  • Assists in communicating with nursing staff and other members of the IDT to gather appropriate skilled documentation
  • Key player in UR/Medicare meeting
  • Attends clinical meetings as appropriate
  • Resolves insurance/managed care operational issues such as authorization and payment level of service
