Requirements of the Registered Nurse (RN) Case Manager :
Must possess a current, unencumbered, active license to practice as a Registered Nurse (RN) in this stateBachelors Degree in Nursing (BSN) from an accredited college or university preferred.Must have, as a minimum, two (2) years of experience with case management skills in a long-term care setting.Proficient in ability to interpret Managed Care / Medicare Advantage Contracts and provide education to the teams in our centers.Knowledge of the various payment models for Managed CareMust have Managed Care / Medicare Advantage billing knowledge and experience.Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.Responsibilites of the Registered Nurse (RN) Case Manager :
Provides communication to the managed care providers regarding a patient’s care and their current level of functioning.Ensures comprehensive care is continued from hospital to skilled nursing centerInsurance updates-with focus on increase in levels and RUGs when appropriateAssists in gathering and reviewing patient records for appeal requestsMedicare Certifications / RecertificationsAssisting with querying the physician for appropriate diagnosis codes for best possible reimbursementAssist in reviewing hospital records for diagnosis codes, treatments and medications.Interviews of the patient when appropriateAssists in communicating with nursing staff and other members of the IDT to gather appropriate skilled documentationKey player in UR / Medicare meetingAttends clinical meetings as appropriateResolves insurance / managed care operational issues such as authorization and payment level of serviceTake pride in your career and join an organization where your talents will shine!
