Position Summary:
Working under the direction of the Wellness Director or scheduled Supervisor-inCharge provides personal care services, assistance, support and supervision to residents. May administer routine medications to residents in accordance with state regulations. Observes changes in residents' conditions and reports to nurse. Regularly checks on resident's safety and well-being, and assists in the preparation of documentation and scheduling of Wellness Staff.
Responsibilities:
- Follow the service plan in providing personal care needs to the residents.
- Assist in the development and updating of resident service plans of health when applicable.
- Assist Director of Nursing with staffing the wellness department, paperwork and coordination of resident physician visits.
- Encourage socialization among residents.
- Promote and assist with activities program, conduct activities as directed. Monitor health, safety and well-being of the residents.
- Assist in keeping the environment safe for residents.
- Provide one-on-one support/visitation to assigned residents as requested. Report concerns immediately about residents to Nurse, and follow-up with any comments in the communication log, complete all shift reports timely. Respond to resident emergencies, provide first aid assistance and arrange for appropriate medical attention/follow up.
- Report any signs of physical or emotional abuse.
- Identify supplies needed to perform tasks and report to the Director-of-Nursing. Carry out all duties assigned by the Director of Nursing or designee.
- Maintain confidentiality of verbal and written information pertaining to residents, facility operations and personnel.
- Staff shifts for Wellness Department, ensure coverage, adjust schedule for both scheduled and non-scheduled call offs.
