Title: Crisis Case Manager
Typical Schedule: Monday - Friday 8am - 5pm

Location of Job: This position will provide services throughout the service area of Spring River Mental Health & Wellness, Inc. in Cherokee County, KS. Primary office is located in Riverton.

Primary Function: To provide case management services to youth and adults in crisis that will:
- Conduct rapid assessments of individuals in crisis, evaluating mental health status, safety risks, and immediate needs.
- Utilize crisis intervention techniques to de-escalate situations and stabilize individuals experiencing acute distress in conjunction with Crisis QMHP.
- Provide immediate crisis counseling and emotional support to clients.
Duties and Responsibilities:
- Connect clients with community resources and support systems to address underlying issues contributing to crisis, including housing, food and medical
- May be required to conduct outreach to clients experiencing crisis in their homes or community setting
- Maintain comprehensive case documentation, including crisis assessments, intervention plans, progress notes, and follow-up documentation
- Collaborate with other CCBHC team members (therapists, psychiatrists, peer specialists) to ensure seamless care coordination
- Timely completion of service notes and other required documentation in the electronic health record
- Provide case management services as indicated by client need and supervisor direction.
- To act as a liaison to law enforcement, the medical community and other referral resources.
- Other duties as assigned by supervisor.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- Excellent communication (verbal and written) and interpersonal skills.
- Knowledge and skill will include trauma informed care, strength-based assessments, and crisis case management.
- Knowledge of community resources to aid the individual/family during crisis.
- Ability to establish professional working relationships with clients, peers and community stakeholders.
- Ability to work independently and as part of a team
- Sound judgment in crisis and routine situations.
- Proficient with computers and Microsoft Office
- Timely completion of EHR documentation and other written documents.
Job Qualifications: BA/BS degree in human service field strongly preferred or be equivalently qualified through a combination of years of relevant experience and education. Posses and demonstrated interpersonal skills, ability to work with children, adults and families experiencing a mental health crisis, and the ability to react effectively in a variety of human service situations. Prior experience should be in mental health or human services field. Successful completion of professional references, background checks, child and adult abuse registry checks, motor vehicle record check and pre-employment drug screen.
NUMBER OF HOURS PER WEEK: 40 - Regular full-time position
Typical Work Schedule: Monday - Friday 8am - 5pm.
Supervised by: Adult Services Director
Hourly wage: $20.30 - $23.32 Commensurate by years of experience and degree.
