Overview
To provide CPST, TCM, and additional rehabilitative services to SPMI adults and/or adults meeting functional criteria in Cherokee County.
Primary Duties and Responsibilities
- To provide quality, medically necessary case management services to adults with a severe and persisted mental illness.
- Ability to meet productivity standards related to the provision of direct service expectations.
- To assure that referrals for CPST or TCM services are acted upon and within 3 business days of referral.
- To assure that clients will be assessed via the strengths/needs assessment tool.
- To monitor and provide timely documentation of all services provided in accordance with policy and SRMHW's accepted standards.
- To attend workshops and complete training requirements as necessary for provision of CPST; TCM; and additional Rehabilitation Services, as determined by supervisor and/or KHS.
- To assist with crisis intervention according to SRMHW’s policy.
- To complete additional duties, as assigned.
Job Qualifications
- BA/BS degree in human service field or be equivalently qualified through a combination of years of experience and education.
- Possess demonstrated interpersonal skills, ability to work with person with severe and persistent mental illness, and the ability to react effectively in a variety of human service situations.
- Prior experience in mental health or human services field.
- Meet specifications outlined by the Division of Healthcare Policy.
- Pass KBI, SRS child and adult abuse registry checks, alcohol/drug screening process, and motor vehicle record check.
Knowledge, Skills, and Abilities
- Educational background and demonstrated knowledge of mental health needs.
- Knowledge of strength based assessments.
- Knowledge of community resources and referral agencies.
- Skilled in establishing and maintaining effective working relationships with clients; staff; and the public.
- Ability to write reports and document client progress.
- Ability to organize and track program evaluation data.
- Ability to utilize computer software efficiently.
Typical Physical Demands and Working Conditions
Requires sitting for periods of time. Occasional bending, stretching or lifting. May require contact with individuals with communicable diseases. Requires the ability to work under high stress conditions for long periods of time. Ability to hear normal range of voice. Frequently walking up and down stairs. Ability to prepare written reports and use telephone and computers. Normal office environment. Occasional exposure to communicable diseases and other conditions. Occasional exposure to aggressive or violent patients. Frequent work in client homes, community agencies and providing transportation of clients for services.

Community Psychiatric Support and Treatment (CPST)
Definition: Goal directed supports and solution-focused intervention intended to achieve identified goals or objectives as set forth in the individualized treatment plan. CPST is a face-to-face intervention with the client present; however, family or other collaterals may also be involved. The majority of CPST contacts must occur in community locations where the child lives, attends, and/or socializes.
Components
A. Assist the client and family members or other collaterals to identify strategies or treatment options associated with the client's mental illness, with the goal of minimizing the negative effects of mental illness symptoms or emotional disturbances or associated environmental stressors which interfere with the client's daily living, financial management, housing, academic and/or employment progress, personal recovery or resilience, family and/or interpersonal relationships, and community integration.
B. Individual supportive counseling, solution focused intervention, emotional and behavioral management, and problem behavior analysis with the client, with the goal of assisting the client with developing and implementing social, interpersonal, self care, daily living and independent living skills to restore stability, to support functional gains, and to adapt to community living.
C. Participation in and utilization of strengths based planning and treatments which include assisting the client and family members or other collaterals with identifying strengths and needs, resources, natural supports and developing goals and objectives to utilize personal strengths, resources, and natural supports to address functional deficits associated with their mental illness.
D. Assist the client with effectively responding to or avoiding identified precursors or triggers that would risk their remaining in a natural community location, including assisting the client and family members or other collaterals with identifying a potential psychiatric or personal crisis, developing a crisis management plan and/or as appropriate, seeking other supports to restore stability and functioning.
Targeted Case Management (TCM)
Components

Targeted Case Management (TCM)
Definition: Targeted Case Management is designed to provide medically necessary services, under a treatment plan approved by the psychiatrist or physician skilled in the treatment of mental disorders and to assist Medicaid beneficiaries in resolving or minimizing the effects of the mental and emotional impairment for which clinical and/or hospital services have previously been provided.
The goal is to enhance independent functioning through which the consumer is integrated into and/or maintained within the community, so that institutionalization is not as likely or frequent. This goal-directed medically necessary service is for adults with severe and persistent mental illness or children with severe emotional disturbance through which the individual is assisted in obtaining access to needed medical, social, educational and other services. All interventions provided shall be related to specific goals set forth in the consumer’s treatment plan. Documentation in progress notes is required for each billed service. Interventions include the following:
Components
A. Treatment Planning: This includes facilitating the team treatment planning process, including documenting the individualized plan, developing goals and objectives based on a strengths assessment, monitoring to ensure that the plan is working, and making changes when needed. It is expected that the treatment planning process include participation of consumers, families, and natural supports, and that documentation reflect this involvement.
B. Collateral Contacts: This includes phone and written correspondence, as well as face-to-face contacts with other social service agencies, schools, housing and employment resources, and medical services.
C. Access to Supports: This includes assisting individuals in obtaining access to needed medical, social, educational, and other services. In addition, the service would assist with applications for benefits, and arrange transportation to needed services.
Salary : $20 - $23
