What are the responsibilities and job description for the Occupational Therapist - Home Health (IPT)/ Springfield position at Springfield Clinic?
The Home Health Occupational Therapist evaluates, plans, and administers comprehensive occupational therapy modalities / regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician.
Principal Responsibilities
- Provides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelines for patients who have debilitating conditions improve the functions of performing everyday tasks in the home.
- Establishes an in-home treatment plan in coordination with the physician and other care team members to develop the daily living skills of their patients and the basic motor functions of patients, as well as to compensate for any loss of function that patient may be experiencing.
- Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
- Provides quality, personalized, evidence-based care, and proven interventions of Occupational Therapy.
- Educates, collaborates with, and conferences with patients, families, and other health care providers; making recommendations for referrals to specialists as needed.
- Supervise Occupational Therapy Assistants and other home health staff as appropriate
- Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines
- Instructs patients and family / significant others in home programs and activities of daily living
