Social Service Worker

SPRINGFIELD VILLA -

In this position you will work closely with the Social Services Director and rest of the Supervisor staff.  We work collectively as a team to achieve the high amount of service possible for our Residents.

List of Job Duties :

• Complete Admission packets
• Assist in Care Plan Meetings
• Provide counseling and support to residents and families.
• Advocate for Resident Rights.
• Facilitate opportunities and concerns between all parties.
• Admission and Discharge Planning
• Collaborate with residents, families, and community resources for extended care.
• Update Care plans and MDS on Quarterly and Annual basis
• Address complaints and greivances
• Communicate effectively with all parties.
• Be POSITIVE
• Be PREPARE
• Be ORGANIZED
