We are looking for a Care Coordinator with strong skills to join our team. You will work with the children of families. You will join a team of Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports. We are looking for someone with the ability to manage a busy schedule, the needs of each family, and documentation needs.
DUTIES:
- Participate in a team-based approach to care by collaborating with integrated disciplines located within Northern Light.
- Maintain records and documentation as required by Northern Light.
- Be familiar with specific outcome measures as required by Northern Light.
QUALIFICATIONS:
BA in related field required. Strong organization and communication skills. Need savvy computer skills to manage multiple systems. All applicants must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving policy.
