Square Medical Group’s (“Square” or “SMG”) mission is to deliver enduring, material improvements in both clinical and wellness outcomes to patients addressing co-occurring challenges with substance use disorder (SUD) and behavioral health (BH). We do this through delivering a specialized continuum of outpatient care of 1,000 units/week to a population of ~5,000 across Massachusetts: medication management, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as three levels of housing (i.e. Medicaid reimbursed Co-Occurring Recovery Residences, self-pay-based sober homes, and state-sponsored low threshold housing).

Program Overview

The residential recovery programs are structured rehabilitation-oriented treatment focused on trauma-informed care and harm reduction. It is designed for motivated individuals who want to enhance the quality of their day-to-day lives. Through active engagement in groups and the development of the community, we will work together to overcome challenges that interfere with our abilities to live life to the fullest.


Residents will be assigned a weekly life skill to ensure ongoing cleanliness/maintenance of the house. Residents are expected to take part in assigned life skills. Staff will help if residents are unsure about how to complete a task. Life Skills are necessary to independent living.

On average, residents spend between three to six months in the three phases of the residential program. Each phase of the program rewards residents with increasing levels of responsibilities and freedoms. Throughout the program, residents are supported by deep expertise from SMG’s outpatient services, our core capability. Residents are also able to stay with our outpatient practice post-discharge for continuity of care.



Position Summary: River House is a 16-bed intensive treatment program for adult men designed specifically for individuals with co-occurring mental health and addiction disorder challenges. As the Residential Clinician, you will be joining a multi-disciplinary team in a beautiful modern setting on the Charles River providing state of the art treatment designed to foster recovery and result in independent living for our residents. Facilitating treatment programming within the COE residential program. The Residential Clinician is key member of a dynamic multidisciplinary team. Responsible for the supervision of two case managers, leading groups, provide family, individual and group counseling, and care coordination services. Qualifications & Education:

  • LADC I, LCSW, CADAC, LICSW or LMHC in state of Massachusetts preferred.
  • 2 years residential or inpatient treatment experience required.
  • Group treatment experience required.
  • Substance use and addictive disorders, co-occurring disorders experience required.
  • Master's degree in social work, counseling or related discipline required.


Responsibilities:

  • Facilitate the COE Residential Program, utilizing evidenced based program curriculum as well as incorporating individual service plan (ISP) goals of residents.
  • Provide compassionate, nonjudgmental clinical treatment and care coordination in accordance with the scope of practice.
  • Complete biopsychosocial assessments
  • Facilitate therapy groups
  • Coordinate care with collaterals and family members as needed
  • Engage with team to provide crisis intervention services as needed
  • Engage in program milieu activities including on-boarding of new residents, drug screens, room searches and community meetings
  • Ensure timely and accurate documentation using the Electronic Medical Record (EMR)
  • Cooperate and work with all members of the COE team to develop, formulate, implement, and revise clinical programming proposals for recommendations to medical and professional staff and to adhere to regulatory body standards and meet program and agency goals and objectives.
  • Work closely with the Program and Clinical Director to achieve the programs financial goals regarding expense management and revenue production, as well as maintain overall administrative and program structure.
