What are the responsibilities and job description for the Case Coordinator position at SRS, LLC?
SRS, LLC is a national network of clinical specialists that perform substance abuse evaluations. We are looking for an entry level community health professional to assist an experienced social worker in managing a hefty case load.
Responsibilities and Requirements for this position include:
- Coordinate and provide telephonic care that is safe, timely, effective, efficient, equitable, and client-centered
- Independently handle case assignments, review service plans and case progress
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help clients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with support team
- Interact with clients to keep track of their progress to assure compliance
- Record case information, review accuracy of all necessary forms and track statistical data for reporting
- Adhere to professional standards as outlined by protocols, rules and regulations
- Previous experience preferably in substance abuse or a related behavioral health field
- Effective communication skills
- Excellent organizational and time management skills
- Problem solving skills and the ability to multi-task
- Compassionate attitude with teamwork skills
- Minimum AA degree in related healthcare field
Job Types: Full-time, Part-time
Pay: $34,500.00 - $38,500.00 per year
Expected hours: 30 – 40 per week
