The RN will assess and coordinate patient's discharge planning needs with members of the healthcare team and may perform point of care testing according to policies and procedures.
Key Responsibilities:
- Document visit notes, treatments, and care.
- Fulfill required visits in a timely manner.
Skills and Qualifications:
- Basic Life Support HealthCare Provider (BLS HCP) certification from American Heart Association (AHA).
- Registered Professional Nurse (RN) licensure from Illinois Department of Financial and Professional Regulation (IDFPR).
