Job Summary:
Coordinates interdisciplinary plan of care through assessment and evaluation of patient condition under the direction of the attending physician. Travel to patient homes or facilities to meet patient needs, assess care requirements, and ensure adherence to home care and/or hospice services policies.
Responsibilities and Requirements:
Primary Responsibilities:
- Assess and coordinate patient discharge planning needs with healthcare team members.
- Perform point-of-care testing according to policies and procedures.
- Collaborate with physicians regarding the plan of care and patient status updates.
- Document status and communication in adherence with regulations.
Education and Experience:
Graduate of accredited school of nursing or education equivalency for licensing. No experience required.
Physical Requirements:
Constant standing, walking, and use of speech. Frequent lifting, sitting, driving, and keyboard use.
Licenses and Certifications:
Basic Life Support Healthcare Provider (BLS HCP) and Registered Professional Nurse (RN) licenses in respective states.
