What are the responsibilities and job description for the PHC Clerk position at St. Benedict's Home Health?
Maintain communication with client, family, physician, caseworker/service coordinator
Document all information regarding client’s care i.e. care plan, task, hospitalizations, out of town and/or complaints, etc.
Communicate with caseworker/service coordinator regarding changes in client care via telephone and/or communication sheet (2067)
Attend daily morning meetings to discuss client and attendant changes
File paperwork in a timely manner
Make sure all pre-initiation activities to client care are done in regards to the time frames set by DADS
Data Entry- client and attendant information into agency’s computer software database
Maintain calendar for visits that need to be done by the PHC Supervisor i.e. supervisory visits, client annual forms and attendant evaluations
Perform annual EMR/NAR verification for all attendants via internet website
Request prior authorizations for personal assistance services
Job Type: Full-time
Expected hours: No less than 40 per week
