We are currently hiring a Full Time Temporary Social Worker to join our Team!
Responsibilities:
This position is responsible for comprehensive assessment and coordination of services required to meet the individual’s psychosocial/mood state, behavioral and discharge needs of each resident. In addition, this position facilitates the interdisciplinary teams’ coordination of resident needs, as well as the development and implementation of a safe and effective discharge plan. Obtains a strong skill set on discharge planning and coordination of external services for patients.
Applicants with rehabilitation, sub-acute experience are encouraged to apply and will be given priority consideration.

Qualifications:
- Bachelor degree in Social Work or related field required.
- Possess a minimum of 2 years’ experience in a health care setting, preferable in a rehab department
- Demonstrated ability to execute discharge planning efficiently in accordance with facility requirements.
- Strong problem solving and organizational skills.
- Sound knowledge of community resources, entitlements, and insurers.
- Strong crisis intervention skills required.
- Excellent verbal and written interpersonal skills that promote family engagement, as well as collaboration across interdisciplinary teams.
