GENERAL SUMMARY:
Provide individual case management services to youth and families enrolled in referred/program services. Schedule/facilitate daily groups/activities for designated youth to meet their needs. Maintain contact with youth’s home, school, and other team members. Provide written updates to the courts regarding youth’s engagement in programming.
ESSENTIAL FUNCTIONS:
- Work with any youth and family that is referred for the duration of their involvement in the program; acknowledge and emphasize the youth and family’s strengths, culture, and community.
- Support the family decision-making process to empower the youth and the family to avoid further law violations or continued involvement in the Juvenile Justice System.
- Prepare and submit thorough documentation according to the program’s designated time frames.
- Presentation of reports to contracted entities such as court officials.
- Professional interactions with school staff to monitor daily school attendance, address school behavior and the performance of each youth to improve the youth’s functioning and behavior.
- Schedule and implement daily programming to address the presenting needs of the enrolled youth.
- Monitor and report attendance and engagement of youth. Create written reports for periodic submission to team members and the court.
- Adheres to St. Charles Youth & Family Services Policies and Procedures.
- Ability to facilitate, counsel and supervise youth and family group sessions.
- Other duties as assigned.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
- High School Diploma required; Bachelor’s degree in Human Services, Social Work, Criminal Justice, Economics, Business or related field preferred.
- Minimum two years’ experience with programs that serve a delinquent or at risk youth population.
- Valid Wisconsin driver’s license and liability insurance.
- Ability to use vehicles for work related duties according to the agency’s insurance liability standards.
- Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
- Knowledge of adolescent and family issues and dynamics.
- Appreciation for cultural and socioeconomic dynamics.
- Knowledge of human growth and development theories.
- Ability to accurately perform record keeping and documentation entries.
- Ability to perform computer tasks using MS Windows applications, agency data base systems, etc.
WORKING CONDITIONS
- Indoor/outdoor requirements (including client homes and community settings).
- Potential for physical intervention of clients as needed.
- Travel approximately 50% using company vehicle.
