We seek a highly skilled and compassionate Registered Nurse Leader to join our team. As an On-Call Registered Nurse, you will play a vital role in organizing and directing hospice care services, ensuring the highest level of patient satisfaction and comfort.

Key Responsibilities
As an On-Call Registered Nurse, your primary duties will include:
Available to receive and respond to calls from Triage during assigned shifts
You will make visits to patients with changing conditions and complete week-end admissions.
Provides comprehensive assessments and ongoing care
You will evaluate patient status, write and initiate plans of care, re-evaluate patient and family/caregiver needs, and revise plans as necessary.
Develops and implements care plans
You will use health assessment data to determine nursing diagnosis, develop a care plan that establishes goals based on nursing diagnosis, and incorporate palliative nursing actions.
Counsels and educates patients and families
You will provide health care instructions to patients as appropriate per assessment and plan, assist patients with activities of daily living, and facilitate their efforts toward self-sufficiency and optional comfort care.
Maintains accurate records and communicates effectively
You will complete, maintain, and submit accurate and relevant clinical notes regarding patients' condition and care given, record pain/symptom management changes/outcomes as appropriate, and communicate with physicians regarding patients' needs.
Requirements and Qualifications
To be successful in this role, you will need:
Graduate of an accredited school of nursing
A minimum of one to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care are required.
Licensure and certification
You must hold current licensure in the State and CPR certification.
Excellent communication and problem-solving skills
You will be responsible for supervising hospice aides and possess excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
Work environment and physical demands
This position requires prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and consideration reaching, stooping, bending, kneeling, and/or crouching.
Preferred Qualifications
A licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order is preferred.
