Job Summary and Qualifications
The Physical Therapist (PT) plays a critical role in our team by managing the complete process of therapeutic care for an assigned group of patients. The PT completes patient assessments, participates in developing and implementing individualized care plans, and provides or directs a majority of the patient's physical therapy to support patients throughout the continuum of care.
- Assesses and evaluates therapeutic/rehabilitative/functional status and participates in the development of the total plan of care.
- Provides purposeful and skilled therapeutic care at patients' homes, using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
- Instructs patients/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Prioritizes caseload, determines daily staffing needs, and coordinates assignments/handoffs.
